- After filling the form online, get print and affix signature on the space given.
- Attach required/relevant documents with the signed form and send it to Punjab Curriculum and Textbook Board as directed in advertisement.
- Please tick mark on the printed form, the documents which have been attached with application form.
- Copy of CNIC of Applicant (Mandatory requirement)
- Original Pay Order/Receipt of Challan form as payment of Review Charges (Mandatory requirement)
- Plagarism Report
- Affidavit (As per given pattern) Sample for Affidavit (Mandatory requirement)
- Hard copy of the Manuscript (Mandatory requirement)
- Softcopy of the Manuscript on CD, DVD or USB
